Instructions:

  1. Fill out the form to print out a prepaid label.

  2. Put your cell phonesandiPads into a box and affix the label to the outside of the package.

  3. Drop your package off at a FedEx location
    -OR-
    Call 405-755-3131 and we will have FedEx come pick up your package.
