After finishing a qualifying non-fiction book, fill out and submit this form to be entered into the weekly drawing. Your entry will be added to the reading program page on our website.

Notice: Books must be entered before 1:00PM on the Friday before the Monday morning meeting. Books entered after this time will be paid on the following Monday.
